office 365 exchange lost email

I have a client in O365 Exchange Online services where they keep loosing internal emails.  They show up on trace log, but fail to land in the users inbox / clutter / junk mail.  I've checked both OWA, and Desktop for rule chain, none there.  Any further suggestions?
